General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to tripping and or falling into unguarded rotating shafts: In the bulk holding tank area located outside the building. Employees at night are not provided with adequate lighting in an area where there are large large holding tanks, and unguarded rotating shafts from electric motors and pumps. This area is poorly lighted and can subject employees to slips trips and falls while attempting to access the treated and untreated water holding tanks. This condition is exacerbated by the unmarked aisle ways in the same area. The lack of adequate lighting and unmarked aisle ways can delay first aid and rescue attempts in the event an employee is injured. Among other methods to correct this condition would be to install adequate lighting to provide a minimum of 10 foot candles of light as recommended by American Nation Standard Practice for Industrial lighting RP-7 for Storage and transfer areas.er
