Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of blunt impact and being crushed while mounting tires on Volvo loader in that: (a) On or about 07/14/04 a tire technician place a hydraulic jack and two jack stands under the rear axle of a Volvo front end loader. The rear axle housing had rounded corners in the areas where the jack were placed. The loader fell off of the jack stands crushing the technician. Wheel chocks were not placed under the wheel to prevent the loader from rolling. No jack stands were used when tire technician and a tire delivery man mounted a tire on the front axle. Manufacturer's instructions for the hydraulic jack could not be located. One feasible and acceptable method, among others, to abate this hazard is to follow manufacturer's guidelines: 1. Use the frame's support points as specified in the "wheel, removing and installing" section, page 12 of the L90E/L120E maintenance manual. 2. Place suitable wedges on both sides of the wheels which are not to be raised off the ground, as specified in "safety when lifting and supporting the machine", page 24 of the L90E/L120E maintenance manual. 3. Ensure employees are familiar with the equipment and the manufacturer's instructions on its operation, maintenance and testing in accordance with ANSI standard titled "Jacks" B30.1-1975 section 1-3-2.(a) "Recommended Operational Procedures".
