were exposed to the hazard of falling from the Holan aerial lift mounted
on International Truck
model 400SER:
(a) Location - Establishment:
On or about 11/24/02, employees were exposed to falling hazards while
operating the
aerial lift.
Feasible and acceptable abatement methods to correct this hazard include,
but are not
limited to the following:
(1) Ensure that a complete periodic inspection is conducted at least
yearly on the Holan
aerial lift by qualified personnel.
(2) Obtain and make available the operator's manual for the Holan aerial
lift,
which has
information, including but not limited to: description, specifications,
and capacities of
the aerial device, operating pressure of hydraulic or pneumatic systems;
instructions
regarding operation and maintenance; and replacement part information, as
required by
ANSI A92.2-1969, Section 6.1.2.
(3) Ensure that operating instruction plate is present, which has
information, including
but not limited to: make, model, and serial number; rated capacity;
platform height;
maximum recommended operating pressure of hydraulic or pneumatic systems;
cautions
or restrictions of operation; operating instructions; and manufacturer's
rated line voltage,
as required by ANSI A92-2-1969, Section 3.1.4.
(4) Provide documentation of the required loading test, insulation tests,
and
manufacturer's electrical tests on insulated devices, as required by ANSI
A92.2-1969,
Section 3.2.3.
(5) Ensure that labels, including but not limited to boom and basket load
limits specified
by the manufacturer, and the attachment point for the body belt lanyard,
are available.
(6) Ensure that the guard over the bucket controls is installed properly.
(7) Ensure that the knob on the upper emergency stop is in place.
