A.  Employer knew or should have known that employee operating a Lull 644
rough terrain forklift, Serial Number JD2633N, on site without wearing the
seat belt was exposed to being thrown from the seat.  A feasible and useful
method to correct this hazard among other methods, is for the operator to
wear
the seat belt.
