A.  Employer knew or should have known that operator not wearing seat belt
in Square
Shooter SS-636C Turbo forklift, Serial Number 002961, at rear of building
constituted a
hazard of being thrown from forklift.  A feasible and useful method, among
others, to
correct this hazard is to assure wearing of seat belt on forklift.
