On or about 06/20/2003, employees were exposed to the hazard of being
struck or
crushed by overhead loads while utilizing hoisting equipment.
Among other feasible methods of abatement, one acceptable method to
correct the hazard
is:
1) Develop and implement a more reliable locking mechanism on the
lowerator bar.
2) Train employees in the safe operating procedures for the trolleying of
overhead loads.
3) Develop and implement a standard maintenance and inspection procedure
of
all
overhead hoisting equipment.
