employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of crushing or impact injuries from powered
industrial truck
accidents:
(a)  Location - Lumber Yard
On or about 8/19/03, employees were exposed to the hazard of being thrown
from the
seat of powered industrial fork trucks they were operating.
One feasible and acceptable method to correct this hazard is to ensure
that operators wear
seat belts at all times while operating powered industrial fork trucks.ial

employees in that employees
were exposed to fall hazards on Cougar Lifts:
(a)  Location - Store Aisles
On or about 8/19/03, employees removed the guard rails from the Cougar
Lift to step
onto the high level storage racks to access kitchen cabinets.
(b)  Location - Store Aisles
On or about 8/19/03, employees transported large items, including, but not
limited to,
bath tubs and doors, that hung over the edge of the Cougar Lift platform.
Among other means, feasible means of abatement would be to:
1.  Design a training program for the Cougar Lift that includes all
aspects of safe
operation for all uses within the store which includes a review of the
operation manual
provided by the manufacturer and a discussion of the warnings included in
the manual
including, but not limited to:
(a)  Ensure that employees do not remove the guard rails when utilizing
the
Cougar Lift.
(b)  Ensure employees do not use the Cougar Lift to transport large items
that hang over
the edge of the platform.
This training should address all of the practical uses deemed appropriate
for this type of
lift specifically related to the needs within the store.
2.  Ensure that all managers who supervise employees who use the Cougar
Lift receive
and understand the training and enforce the safe operating principles.
