conjunction with a
descent device while cleaning windows.
(a) Location - 700 Technology Square, Cambridge, MA:
On or about 12/01/2003,  employees were exposed to fall hazards while
using a rolling
outrigger in conjunction with a descent device.
Among other means, feasible means  of abatement would be to:
1) Ensure when using one anchorage for tieing back both the rolling
outrigger and the
descent device that the anchorage is capable of withstanding a 10,000 lb
load.
2) Ensure the primary line, life line, and tie back line are attached with
a
minimal of
slack and within fifteen degrees of perpendicular.
