Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling while using a rolling outrigger in conjunction with a descent device while cleaning windows. (a) Location - 700 Technology Square, Cambridge, MA: On or about 12/01/2003, employees were exposed to fall hazards while using a rolling outrigger in conjunction with a descent device. Among other means, feasible means of abatement would be to: 1) Ensure when using one anchorage for tieing back both the rolling outrigger and the descent device that the anchorage is capable of withstanding a 10,000 lb load. 2) Ensure the primary line, life line, and tie back line are attached with a minimal of slack and within fifteen degrees of perpendicular.
