Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being injured by an explosion. (a) Location - Manhole at corner of Elm and Woodlawn Streets, Everett, MA: On or about 03/09/2004, employees were exposed to the hazard of being injured by an explosion from a build up of flammable or explosive gas within the manhole. Among other methods, feasible means of abatement would be to develop and implement a confined space entry program that meets the requirements of OSHA 1910.146 or ANSI Z117.1-1989 or newer. The program should include, but not be limited to: 1) Air testing/monitoring prior to entry and continuously where there is potential for hazardous atmospheric conditions. 2) Proper venting of confined space areas with fresh air intake and exhaust systems. 3) Effective training of the authorized entrants, attendants, and all affected employees. 4) Strict enforcement of developed and implemented policies and procedures.
