Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of crushing injuries on a platform attached to
a powered industrial
truck:
(a) Location - Warehouse:
On or about 4/6/04, employees were exposed to being crushed when the
platform on
which they were standing, filled with file cabinets, tipped the powered
industrial truck
elevating it.
Among other means, feasible means of abatement would be to:
Implement specific operating procedures for use of the platform, including
the following
work rules:
1.  Men and materials may not be transported at the same time;
2.  The weights of the men may not exceed the calculated weights allowed
using
Appendix A of 1910.178 and ANSI/ASME B56.1-1993;
3.  All personnel must stand to the truck side of the load center of the
fork truck;
4.  Material, when loaded, will be loaded as far to the face of the forks
as possible;
5.  The platform will have a marking six inches in from the side edges and
nothing may
be placed, nor shall employees stand, outside of this marking;
6.  The platform will be provided with load rating signs;
7.  Each employee will be trained on the safe load allowances and how to
accurately
determine the weights of each piece and how to place them.
