Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard: (a) Location - 4A Gill Street, Woburn, MA On or about 6/18/04, an employee was crushed between the Skyjack scissors lift and the door jamb while walking the lift from the main room to the loading dock area. Among other means, one feasible means of abatement is for the employer to develop and implement a training program for scissors lift operators.
