Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of burns or explosions while operating an oxy acetylene burning outfit with out the use of flash back arrestors. (a) Location - 1360 Broadway, Saugus, MA: On or about 08/11/2004, an employee was not protected by a back flash arrestor when performing burning and cutting of steel. Among other means, Feasible means of abatement would be to; 1. Install anti back pressure devices on the oxygen and acetylene lines per ANSI Z49.1
