What this inspection record means

OSHA opens inspections for many reasons — routine scheduling under a national or local emphasis program, an employee complaint or referral, or a follow-up after a reported injury. Opening or conducting an inspection is not itself an allegation or a finding that this employer broke any rule; any findings appear as the citations listed below, and citations can be contested, reduced, or withdrawn.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
Hazards created by the use of elevated work platforms which were not
designed, constructed
and/or used in accordance with the acceptable requirements of the American
National Safety
Standard for Rough Terrain Forklift Trucks, ASME B56.6-1992. The
personnel platform and
its use did not meet the design requirements of that standard in the
following areas: A trained
operator was not in position to control the rough terrain forklift or
available to operate controls.
When the operator was not in the operating position, the truck wheels were
not blocked with the
parking brake applied and all travel controls in neutral. These
conditions existed when an
employee was working on a residential building from a platform attached to
the
forks of a rough
terrain forklift. Among other methods, one feasible and acceptable
abatement method to correct
these hazards is to assure that the design, manufacture and use of all
attachments for Rough
Terrain Forklift Trucks are in accordance with the most current American
National Standard,
B56.6 (presently B56.6-1992).
An employee was working from a platform attached to a rough terrain
forklift and no one
was at or near the controls of the lift and the wheels were not chocked.
