North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (Ref: American National Standard (ANS) A10.14-1991 para 4.3.3.4, 4.3.3.7,6.2.1.1.5, 6.3.2(e) a)facility, dryer #1, fall from height - documentation by qualified engineer that horizontal lifeline was capable of supporting static load of 5000 pounds did not exist. facility, dryer #1, fall from height - Anchorages for horizontal lifeline was not specified by a qualified person. c)facility, dryer #1, fall from height - employees connected snap hooks back to its integral lanyard. facility, dryer #1, fall from height - horizontal lifeline used for anchorage point was not located above wearers harness attachment.
