were exposed to the hazard of falling to the ground while working in an
unsecured platform
raised in the air by the forks of a rough terrain powered industrial truck:
(a)  On or about 1/29/03, at Eckerd construction site, 8520 Transit Road,
Amherst, NY:
An employee was securing weatherproofing while in a "cage-type"
workplatform
supported in the raised position by the forks of a CAT Rough Terrain
Forklift Truck.
The workplatform was approx. 15 ft. above the ground and was not secured
to the truck.
Among others, a feasible method to eliminate or materially reduce this
hazard is to:
1. Install properly sized chains to secure the workplatform to the
forklift truck.
