Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed in a forklift rollover: a)The employer did not ensure that employees operating forklifts use the forklift operator restraint system (seatbelts). Feasible and acceptable abatement methods include, but not limited to ensuring that employees utilize the seatbelt while operating the forklifts.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burn hazard: a)The employer did not ensure that steam pipes in the washer area were covered to prevent employees from making contact with the bare pipes. Among other methods, one feasible and acceptable method to correct the hazards is to insulate each pipe to prevent employee contact.
