without the use of an approved work platform.
1) Instruct each employee during initial training and safety meetings, the
important proper fall
protection procedures and the use of properly designed work platforms when
lifting personnel.
2) Conduct periodic monitoring inspections of job site operations and
correct deficiencies
immediately.
On or about 03/25/04 at 5595 Peachtree Ind. Blvd., Doraville, GA:
a) Employee was exposed to a fall hazard. Employee was elevated to
approximately 21' to nail
2x6 boards on the top side of a concrete wall face. The employee was
elevated by a LULL
Rough Terrain Forklift while standing on an unsecured wooden pallet and
not in an approved
man basket.d
