29 CFR 1926.21(b)(2):  The employer did not instruct each employee
in the recognition and avoidance of unsafe condition(s) and the
regulation(s) applicable to his work environment to control or
eliminate any hazard(s) or other exposure to illness or injury:
(a)  On or about May 26, 2003  and at times prior,
employees were exposed to amputation injury while
operating the Central Machine Shear without receiving
appropriate safe operating instructions for their
personal protection.
and/or in the alternative:
Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to amputation injury. from lack of training
on correct operating procedures for personnel involved in shearing
operations as Reference in ANSI B11.4-1973, Section 7.1.4 which
states "Instruction of Operators. The employer shall train and
instruct the operator in the safe method of work before starting
work on any operation covered by this standard. The employer shall
enforce by adequate supervision the use of correct operating
procedures by personnel involved in shearing operations."
(a)  On or about May 26, 2003  and at times prior,
employees were exposed to amputation injury from lack of
training on correct operating procedures for personnel
involved in shearing operations as Referenced in ANSI
B11.4-1973, Section 7.1.4 which states "Instruction of
Operators. The employer shall train and instruct the
operator in the safe method of work before starting work
on any operation covered by this standard. The employer
shall enforce by adequate supervision the use of correct
operating procedures by personnel involved in shearing
operations."
NOTE:  AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS TO ENSURE THAT OPERATORS RECEIVE
TRAINING ON THE CORRECT OPERATING PROCEDURES BY PERSONNEL INVOLVED
IN SHEARING OPERATIONS.
