Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injury from collapse of the crane: (a) On or about March 4, 2004 and at times prior, employees were exposed to crushing injury while using a National series 800 telescoping truck mounted crane in which the outriggers were set upon blocks that were not uniform or adequate in size to prevent unintended tipping or collapse of the crane, as referenced in ASME B30.5-2000. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter"). "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS: 1. Utilize the calculation method for determining the proper size, and length of the blocking/cribbing area by determining the area of the float (in square feet) x 3 which will equal the blocking area (in square feet). 2. Utilize the calculation methods of capacity of the crane (in tons) divided by five which will equal the blocking area in square feet.
