PRESS BRAKE DEPARTMENT; THE AMADA RG-35, AMADA RG-50 AND THE HURCO PH 40-20 PRESS BRAKES WHICH WERE EQUIPPED WITH FOOT TREADLE OPERATOR CONTROLS WERE NOT PROVIDED WITH A MEANS TO PREVENT UNINTENTIONAL MACHINE ACTIVATION.AL

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to hazards posed by inadequate press brake operation training. a) PRESS BRAKE DEPARTMENT; THE EMPLOYER HAD NOT DEVELOPED AND IMPLEMENTED A WRITTEN TRAINING PROGRAM TO ENSURE THAT THOSE EMPLOYEES PERFORMING PRESS BRAKE OPERATIONS, INCLUDING TOOLING INSTALLATION,SET UP ADJUSTMENTS, AND LOADING, BENDING AND UNLOADING OF WORKPIECE COMPONENTS ARE PROPERLY TRAINED IN SAFE WORKING PROCEDURES AND ARE QUALIFIED TO PERFORM FUNCTIONS TO WHICH THEY ARE ASSIGNED.
