Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were causing or likely
to cause death or serious physical harm in that employees were exposed to
falls in excess of 15 ft when riding
and working from an elevated fork truck without any fall protection in
place. One feasible and acceptable
method to correct this hazard is to attach and secure an approved safety
work platform to the forks of the
truck :
A) LOCATION: WEST SIDE OF JOB SITE, 1455 NORTH MAIN ST., PALMER, MA
AN EMPLOYEE WAS OBSERVED RIDING AND WORKING ON THE FORKS OF A LULL WITHOUT
A SECURED SAFETY PLATFORM.  THE EMPLOYEE WAS OBSERVED RIDING ON THE FORKS
WHILE THE LULL WAS IN MOTION AND ALSO BEING LIFTED TO UPPER LEVELS OF A
SCAFFOLD STRUCTURE.
