employee, conditions of
employment and a place of employment free from recognized hazards which
caused or were
likely to cause death or serious injury to employees:
On September 22, 2003, employees were using a Champ forklift (Model
6425HLP-225,
S/N CC724825) with a homemade personnel platform.  The personnel platform
was not
designed according to the specifications stated under ASME B56.6-1992 for
use as a
personnel platform.

On September 22, 2003, an employee was working from a personnel basket
without a
body belt and lanyard.  He was to release and remove a sling from a new
parking lot
light pole that had just been set in place and secured.
