Minn. Stat. 182.653 subd. 2:  The employer did not furnish to each
employee, conditions of
employment and a place of employment free from recognized hazards which
caused or were
likely to cause death or serious injury to employees:
An employee was exposed to fall hazards in excess of 15 feet while working
from a
personnel basket, which was attached to forks of a Sky Trak 6036 forklift.
The
employee was not wearing any personal fall arrest equipment.
