40.1-51.1(a) of the Code of Virginia:  The employer, Silver Lake Welding
Service, Inc., did not furnish a place of
employment which was free from recognized hazards that were causing or
likely to cause death or serious physical
harm to employees in that an employee was operating a Pacific Press Brake
located in Building 3, which exposed the
operator to:
Amputation injuries by not providing effective machine guarding for the
point of operation or ensuring that the two
hand controls are operable.
The operator was bending materials which range in size, the smallest being
is 5" x 5", which places his hands very
close to the point of operation.
One feasible and acceptable abatement method to correct this hazard, among
other methods, would be to repair the two
hand controls to ensure that the operators' hands are not near the point
of operation when the press brake is activated
with the foot control such as that called for by ANSI B11.3-1982, American
National Standard Safety Code for Power
Press Brakes.
