Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by the load:
a)Throughout the facility, on or about 3/25/03:  The following hoists were
not
being inspected on a frequent and periodic basis:
1)Pre-Heat Area, 2-Ton Coffing Hoist, Model Number EC.4008.3.
2)Extrusion Area, 2-Ton CM Hoist #1, Model Number RR 2.
3)Extrusion Area, 2-Ton CM Hoist #2, Model Number RR 2.
4)Clean Room #1, 2-Ton Coffing Hoist, Model Number EC.4016.4.
5)Clean Room #1, 1-Ton Coffing Hoist, Model Number EC.2016.3.
6)Clean Room #2, 2-Ton Coffing Hoist, Model Number EC.4008.4.
Among other methods, one feasible method of abatement is to conduct
frequent and
periodic inspections of the hoists for wear, deterioration and malfunction
as per
ASME B30.16-1993
b)Throughout the facility, on or about 3/25/03:  The support structures
for the
following hoists had unknown load capacity ratings:
1)Extrusion Area, 2-Ton CM Hoist #1, Model Number RR 2.
2)Extrusion Area, 2-Ton CM Hoist #2, Model Number RR 2.
3)Clean Room #1, 2-Ton Coffing Hoist, Model Number EC.4016.4.
4)Clean Room #1, 1-Ton Coffing Hoist, Model Number EC.2016.3.
5)Clean Room #2, 2-Ton Coffing Hoist, Model Number EC.4008.4.
Among other methods, one feasible method of abatement is to inspect the
support
structures for the hoists as per ASME B30.16-1993, to ensure the structure
is
designed to withstand the loads and forces imposed by the hoist for the
rated load.
c)Pre-Heat Area, on or about 3/25/03:  Below the hook lifting device had
unknown load capacity rating and was not being inspected on a frequent and
periodic basis.
Among other methods, one feasible method of abatement is to conduct
frequent and
periodic inspections of the lifting device and mark the lifting device
with the rated
load capacity as per ASME B30.20-1993.
Abatement certification must be submitted for these items.
