Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the overhead hoist, below-the- hook lifting devices or bridge crane system fail: a)Throughout the facility, on or about 4/14/03: Hoists were not being inspected on a frequent or periodic basis. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: Conduct frequent and periodic inspections of the hoists for wear, deterioration and malfunction as per ANSI B30.16-1993 Section 16-2.1. b)The following overhead hoists had broken or missing safety latches on the lifting hooks: 1.Steel Shop, Bay 2, on or about 4/14/03: A Cleveland 1 ton hoist, serial #130161 had a missing safety latch on the lifting hook. 2.Steel Shop, Storage Area, on or about 4/14/03: A Cleveland 1 ton hoist, serial #130162 had a missing safety latch on the lifting hook. 3.Glue Room, on or about 4/14/03: A CM Meteor 3 ton hoist, serial #WS-011HY had a missing safety latch on the lifting hook. 4.Glue Room, on or about 4/14/03: A CM Meteor 3 ton hoist, serial #WS-013HY had a missing safety latch on the lifting hook. 5.Glue Room, on or about 4/14/03: A CM Meteor 3 ton hoist, serial #WO361A had a missing safety latch on the lifting hook. 6.Glue Room, on or about 4/14/03: A CM Meteor 2 ton hoist, serial #WO-556M had a missing safety latch on the lifting hook. 7.Finishing Department, on or about 4/14/03: Hoist, serial #128264 had a missing safety latch on the lifting hook. 8.Finishing Department, on or about 4/14/03: Three hoists, serial #124148, serial #HRA64413 and serial #128261 had missing safety latches on the lifting hooks. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: Install new safety latches on the lifting hooks in accordance with ANSI B30.16-1993 Section 16-1.2.9. c)Glue Room, on or about 3/31/03: A below-the-hook basket lifting device used for lifting clamps had no rated load capacity marking. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: Mark the rated load of the lifting device on the main structure or equivalent in accordance with ASME B30.20-1993, Section 20-1.2.1. d)The wire rope ends on the following hoists were not secured to the hoist drum when the hooks were lowered in the extreme low position: 1.Steel Shop, Bay 2, on or about 4/14/03: The wire rope end for a Cleveland hoist, serial #130161 was not secured to the hoist drum. 2.Steel Shop, on or about 4/14/03: The wire rope end for a Cleveland hoist, serial #130162 was not secured to the hoist drum. Among other methods, one feasible and acceptable abatement method to correct thishazard is to: Attach the rope ends to the hoist in a manner to prevent disengagement throughout rated hook travel in accordance with ANSI B30.16-1993, Section 16-1.2.6. e)The supporting structures for the following hoists were underrated for what the hoist capacity was rated for: 1.Glue Room, on or about 4/14/03: A CM Meteor hoist, serial # WS- 011HY rated for 3 tons was mounted on a bridge crane structure which was rated for 2 tons. 2.Glue Room, on or about 4/14/03: A CM Meteor hoist, serial # WS- 013HY rated for 3 tons was mounted on a bridge crane structure which was rated for 2 tons. 3.Glue Room, on or about 4/14/03: A CM Meteor hoist, serial # WO- 361A rated for 3 tons was mounted on a bridge crane structure which was rated for 2 tons. 4.Glue Room, on or about 4/14/03: A CM Meteor hoist, serial # WS- 012HY rated for 3 tons was mounted on a bridge crane structure which was rated for 2 tons. Among other methods, one feasible and acceptable abatement method to correct thishazard is to: Prior to installation and use, ensure the supporting structure is designed to withstand the loads and forces imposed by the hoist for the rated load in accordance with ANSI B30.11-1980, Section 11-2.2.2. Abatement certification must be submitted for Instance a, b, d and e.d
