hoist, below-the-
hook lifting devices or bridge crane system fail:
a)Storage Room, on or about 4/15/03:  An overhead chain hoist was not being
inspected on a frequent or periodic basis.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to:
Conduct frequent and periodic inspections of the hoists for wear,
deterioration and malfunction as per ANSI B30.16-1993 Section 16-2.1.
b)Storage Room, on or about 4/15/03:  An overhead chain hoist had a missing
safety latch on the lifting hook.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to:
Install new safety latches on the lifting hooks in accordance with ANSI
B30.16-1993 Section 16-1.2.9.93
