Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to A FALL HAZARD: a)At the facility, on or about 09/11/03: Employees were cleaning two holding tanks, the"blue" tank had an elevation of ten feet, the "silver" tank had an elevation of twelve feet. The employees stand on moveable grating on top of the tanks and used high pressure water to rinse the interior. One employee used a harness attached to a nine foot lanyard which was looped around the anchorage point and attached to itself. The other employee used a harness attached to six foot lanyard which was looped around the anchorage point and attached to itself. Among other methods, several feasible and acceptable abatement methods to correct these hazards are to: 1.Provide lanyards and ensure personal fall arrest systems (PFAS) are rigged to prevent a free fall in excess of six feet in accordance with the American National Standards Institute (ANSI) Z359.1 - 1992 document, paragraph 7.2. 2.Ensure lanyards have connectors which are compatible with the anchorage points so that the connector attaches to the anchorage point and not back onto itself in accordance with ANSI Z359.1 - 1992, paragraph 7.2.5. Abatement certification must be submitted for this item.
