employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to THE HAZARD OF BEING STRUCK-BY THE LOAD/EQUIPMENT:
a)Throughout the facility, on or about 10/20/03 and 10/23/03:  The support
structures for the following hoists had unknown load capacity ratings:
1)Casing Area, 1/2-Ton CM Hoist.
2)Wipes Line, Two 1/2-Ton CM Hoists.
3)Grease Unloading Area, 1/2-Ton CM Hoist.
4)PS Unloading Area, 1/2-Ton CM Hoist.
Among other methods, one feasible method of abatement is to inspect the
support
structures for the hoists as per ASME B30.16-1993, to ensure the structure
is
designed to withstand the loads and forces imposed by the hoist for the
rated load.
b)Throughout the facility, on or about 10/20/03:  The following
below-the-hook
lifting devices had unknown load capacity ratings and were not being
inspected on a frequent and periodic basis.
1)Casing Area, 1/4-Ton CM Hoist, (2) below-the-hook lifting devices.
2)Wipes Line, 1/2-Ton CM Hoist, (2) below-the-hook lifting device.
3)Casing Area, 1/2-Ton CM Hoist, below-the-hook lifting device.
Among other methods, one feasible method of abatement is to conduct
frequent and
periodic inspections of the lifting device and mark the lifting device
with the rated
load capacity as per ASME B30.20-1993.
Abatement certification must be submitted for these items.
