Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to being struck by flying debris and ammonia caused by a
pressure vessel
explosion:
a. On or about 01/09/2004, in the ammonia compressor room and the
ammonia receiver room: Pressure relief devices installed for the protection
of pressure vessels including but not limited to the main receiver, were
not
replaced every 5 years.
Abatement note: Among others one feasible abatement method to correct this
hazard is to follow the International Institute of Ammonia Refrigeration
(IIAR) Bulletin 110 inspection and maintenance of ammonia refrigeration
systems. This document requires that pressure relief devices be replaced
every 5 years with devices selected in accordance with ANSI/ASHREA-15.
b. On or about 01/09/2004, in the ammonia receiver room: The main
ammonia receiver which exceeds 10 cubic feet in capacity was equipped with
only a single pressure relief device.
Abatement note: Among others one feasible abatement method to correct this
hazard is to follow the ANSI Standard for mechanical refrigeration B9.1-
1971 sec 10.4.2.2. This document requires that all pressure vessels with a
capacity of greater than 10 cubic feet be equipped with parallel pressure
relief devices.
c. On or about 01/09/2004, in the ammonia compressor room: The ammonia
liquid accumulator, a pressure vessel exceeding 3 cubic feet capacity and
which can be shut off by block valves, was not equipped with a relief
device.
Abatement note: Among others one feasible abatement method to correct this
hazard is to follow the ANSI Standard for mechanical refrigeration B9.1-
1971 sec 10.4.2. This document requires that all pressure vessels with a
capacity of greater than 3 cubic feet be equipped with pressure relief
devices.
Abatement certification must be submitted for this citation.
