supervision,instruction,and testing
to ensure that the Terex RT 230 hydraulic crane was operated within the
manufacturer's limitations; specifically the employer failed to ensure
that the
operator did not elevate the boom to an elevation greater than 60 degrees
while
the outriggers were retrated and the boom was not centered over the cab as
required by the cranes operating instructions.
