Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire: a) Exhaust Duct for Kettle 240C - Employees were exposed to a fire that was started when accumulated charcoal dust in the duct and hydrogen generated from an acid leaching pit were ignited by platinum and/or palladium. On or about March 15, 2003. Feasible Abatement Method: 1. Install fire suppression equipment, such as, but not limited to, a sprinkler system, inside the ducts. 2. Schedule frequent cleaning of interior duct work surfaces to minimize the accumulation of combustibles.
