employment and a place of employment which were free from
recognized hazards that were causing or likely to cause death or
serious physical harm to employees in that employees were exposed
to:
a)on or about 13 Feb 03, wall seperating dust bin #1 and
boiler room, penetrations in wall allowed for fire spread
from the boiler room to dust bin #1 ultimitely resulting
in an explosion in a dust collection system.
Reference NFPA 664-1962, Section 1023:  Adequate precautions
shall be taken to prevent the spread of fire form one section
of the plant to another.
