Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck/crushed by equipment during oil
and gas well drilling
operations.
a)  Caza Drilling, Rose Ranch 11-12B sec10, 7s,93w, Rifle, CO:  The
employer did not adequately train and instruct each employee on safe work
practices including the recognition and avoidance of unsafe conditions
associated
with assigning new personnel to operate the drilling console on Rig 11.
On or
about 4/30/2003 an employee was seriously injured after being struck by
pipe
tongs that were inadvertently actuated by an inexperienced employee
operating the
drilling console.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard is implement an effective safety program which specifically
outlines
methods and procedures for  all employees whenever inexperienced personnel
are
assigned to new positions. Observation of new personnel work performance
should be maintained until the applicable supervisor is satisfied that the
personnel
can fill the position in a safe and effective manner as found in the
American
Petroleum Institute, API RP54 1999, Section 6.

5/01/03, the auxiliary escape line attached to the "monkey board" on Rig
11 was
obstructed due to a parked vehicle in the landing area. Condition exposed
employee to a hazard of colliding/striking a parked vehicle.
Among other feasible and acceptable abatement methods to abate this hazard
is
to ensure that all escape line routes are kept clear of obstructions as
referenced
by American Petroleum Institute, API RP 54 1999 in section 6.10.1.

were exposed the hazard of falling while using a derrickman's escape line:
a)  Caza Drilling, Rose Ranch 11-12B sec10, 7s,93w, Rifle, CO:  On or about
5/01/03, the derrickman's escape line leading from the "monkey board" on
Rig
11 was tied to an inadequate anchor point below. Condition exposed the
would
be user to a fall hazard as the anchorage point was not capable of
supporting the
anticipated load.
Among other feasible and acceptable abatement methods to abate this hazard
is
to ensure that the ground anchor point is able to withstand a pull of at
least 3,000
pounds as found in the American Petroleum Institute, API Recommended
Practice
54, 1999 in section 6.10.1
