Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish a place of employment which was free from recognized hazards that
were causing or
likely to cause death or serious physical harm to employees in that
employees were exposed
to struck by hazards:
(a)Rebar Placing Inc. at 136th Avenue and I-25, Broomfield, CO 80260: On or
before 08/29/03, the employer did not ensure that 5 employees reached the
I-25
median safely.  Five employees were observed crossing I-25 on foot, from
the east
side to the median, across three lanes of high volume, high speed traffic.
Condition
exposed employees to the hazard of being struck by moving vehicles.
Abatement Note:  Abatement certification and documentation are required
for this item (See
enclosed "Sample Abatement-Certification Letter").
Among other methods, one feasible and acceptable abatement method included
using the
extended cab pickup, which was on-site, to bring and return employees
to/from the median.
Parking sufficient for several vehicles was provided at the median, with
protection provided
by cement bunkers.

likely to cause death or serious physical harm to employees in that
employees were exposed
to falls and struck by hazards:
(a)Rebar Placing Inc. at 136th Avenue and I-25, Broomfield, CO 80260:  On
or
before 08/29/03, the employer did not provide safe egress from a job site
located in
the middle of I-25.  Five employees rode in the bed of an extended cab
pickup truck.
They were not sitting in secure seats and were not wearing seatbelts, and
the truck
bed was full of supplies, resulting in raised, uneven surfaces.  The
employer
merged
the truck into high-speed, high-volume traffic.  In addition, the truck
traveled over
rough, newly excavated surfaces upon re-entering the job site.   Condition
exposed
employees to the hazards of falls and of being struck by moving vehicles.
