Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish to each of his employees employmnent and a place of employment
which were free
from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that employees were exposed to:
(a) Industrial Container Services Inc., at 640 Baseline Rd., Brighton, CO:
On or
before January 16, 2004, the employer did not ensure that the guardrails
at the bend
in the de-denter processing line were installed, thereby exposing four or
nore
employees to a crushing hazard from falling drums.
Abatement Note: Among other methods, one feasible and acceptable method to
correct this
hazard is to re-install the missing guardrails as required by ASME
B20.1-2000 5.9.1.2.

for this item (See
enclosed "Sample Abatement-Certification Letter").
