a)Mooretown Gym - An employee was exposed to a 25 foot fall hazard while
operating a
Skyjack lift without the safety chain being attached on or about 8-18-03.
ABATEMENT NOTES:
Among other methods, one feasible method to reduce the hazard is to:
1)Train the operator to comply with ANSI/SIA  standard A92.6-1999 section
8.8(2). which
states that before each elevation of the platform, the operator shall
ensure that guardrails
are installed and access gates or openings are closed per the
manufacturers
instructions.
