Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazards of blast overpressure and/or fragmentation
created by a detonation
of explosives:
Tree Farm:  The top ring of the Crimping Machine blew off when a blasting
cap detonated
during the assembly operation.
The following are feasible and acceptable methods to abate the hazard:
Design the crimping machine so that if a blasting cap detonates inside the
crimping machine
during assembly, the machine safely limits the blast overpressure and
safely limits the energy
level of any fragments created.
Test the crimping machine prior to use under conditions that simulate a
credible
event (worst
single event that is likely to occur) plus 25overcharge to confirm that
it can safely withstand
a blasting cap detonation.
Redesign the electrical/pneumatic control circuit for the crimping machine
so that it has control
reliability to ensure the machine will fail to a safe mode (i.e. will not
operate) if the electric
micro switch that senses the full insertion of the blasting cap does not
operate properly.
