Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: Company: Seat-belts were not provided on the two Hyster E120 powered industrial trucks. The trucks were operated on an inclined parking lot surface, on a ramp and attachements such as boom extensions and self-dumping hoppers were used on the vehicles. Among others, one feasible and useful method to correct this hazard is: Contact the powered industrial truck manufacturer to have seat-belts installed on the two Hyster E120 vehicles and require that employees wear them while operating the vehicles.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling loads: Grainer Area: The wire rope provided on the LO-HED monorail crane, serial number A35236A-2, had excessive wear. Also, the pendant controls were not legible, the hook was worn and slightly twisted and the monorail was not marked with the load capacity. Among others, one feasible and useful method to correct this hazard is: 1.Conduct monthly, documented inspections of the running rope and hook. Replace hoist components as needed. 2.Ensure that all crane components are properly marked and are legible.
