Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and/or explosion hazards: A.Maintenance Area: Two gas fired heaters were used to heat parts for maintenance purposes. The natural gas supply came in over the department in a two inch black pipe. A hose had been attached to the pipe that was approximately 25 feet long. The hose led down to a splitter. One hose was 14 feet long and it led to the heater at the entrance to the department; the hose was run behind a desk and equipment. The second hose was 20 feet long and led to the heater at the back of the department. Among other methods, one feasible and acceptable abatement method to correct this hazard is: Install hard-piping, flexible hose and shut-off devices in accordance with the National Fire Protection Association standard 54, National Fuel Gas Code. B.Flammable Storage Room: The door connecting the Paint Bay to the Flammable Storage Room did not move freely or with a reasonable amount of force. It was left open during the work shift. Also, the Flammable Storage Room was not provided with continous mechanical ventilation. Class 1 flammable liquids were dispensed in the room. Among other methods, one feasible and acceptable abatement method to correct this hazard is: Ensure that the access door is in proper working order and is self-closing and maintain continuous ventilation in the room.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: The following powered industrial trucks were not provided with seatbelts: A.Orange Yale. B.Green Yale, FT 29. Among other methods, one feasible and acceptable method to correct this hazard is to: Install seat-belts on all powered industrial trucks that pose a tip-over hazard. Instruct employees in the use of the restraint system and require that they be worn at all times while operating the vehicles.
