Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed or struck by: (a)Freightliner roll off truck (vehicle number 7FUYZCBY8LH410723) equipped with the ferrous bed roll off: Employees worked in the yard where the roll off truck was operated in reverse gear without a back up alarm. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install a back up alarm which is distinguishable from the surrounding noise level. Abatement certification and documentation are NOT required for this item.
