Loader, serial number DW644ED518047, articulated loader was operated
without the seat belt being used by the operator.
(b) On 15 July 2004, and at times prior there to, the Hyster lift truck
model
H80XL, S/N E005003750L, powered industrial truck (forklift) was operated
without the seat belt being used by the operator.
"AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE
ABATEMENT METHOD TO CORRECT THIS HAZARD  IS TO REQUIRE ALL
EMPLOYEES USE SEATBELTS WHEN OPERATING EQUIPMENT WITH
SEATBELTS."
