recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazards of falling from  the Hytrol conveyor, being
caught between the
small portable conveyor and being struck by boxes on the main conveyor due
to the unexpected
startup of the main conveyor while either reaching onto belt, kneeling on
the belt and standing
on the belt to unjam boxes:
(a)On or about May 1, 2003, employees reaching onto and standing on the
Hytrol conveyor
to remove jammed packages were only utilizing the normal stop button
located at the Splitter
stations without an operable audible alarm system for the conveyor.
Among feasible abatement methods to correct the hazard is to ensure that
each employee
involved in the operation of loading and unloading packages have the
ability
to lockout at the
nearest emergency stop button to prevent the intentional startup of the
conveyor after situations
such as unjamming packages.
