Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to being struck by a high pressure flowline due to failure to
adequately secure the
flowline:
a) Jeffery Mealy Well#2, Green Township, Mercer County,Jamestown, Pa. The
flowback line
had not been secured at the wellhead, at the 90 degree turn where the 2
inch line was connected,
and at the discharge into the flowback tank.
Among other feasible and acceptable methods to correct this hazard are to:
1. Secure the wellhead piping to prevent rotation in the event of pipe
failure.  ( Ref: American

2. Secure the discharge piping at the 90 degree turn where the 2 inch line
was
connected.

3. Secure or anchor the flow line at the discharge end of the flow
line/flowback tank and at
intermediate intervals along the line of each 15 to 20 feet.  ( Ref:
