Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to hazards of falling loads due to hoist component failure:
a)Welding and Repair Shop, Union Drilling, Punxsutawney, PA -
Employee(s) using a CM Lodestar Protector Hoist, serial #L-71807, and
shop made "A" frame support to lift steel pieces.  The rated load was not
permanently marked on the hoist or load block and clearly legible from
the operating position.
Among other methods of abatement, one feasible and acceptable method to
correct this
hazard is to:
1)Rate and mark hoist and support as per paragraph 16-2.1.1.1,
ANSI Standard B30.16, Overhead Hoist Standard.
b)Welding and Repair Shop, Union Drilling, Punxsutawney, PA -
Employee(s) using a CM Lodestar Protector Hoist, serial #L-71807, and
shop made "A" frame support to lift steel pieces. Hoist did not have a
legible label displaying the following information concerning safe
operating procedures:
1)The word WARNING or other legend designed to bring the label
to the attention of an operator.
2)Cautionary language against:
a.Lifting more than rated load.
b.Operating hoist when hook is not centered under hoist.
c.Operating hoist with twisted, kinked or damaged chain or
rope.
d.Operating damaged or malfunctioning hoist.
e.Operating a rope hoist with a rope that is not properly
seated in it's groove.
f.Lifting people and loads over people.
g.Removing or obscuring warning label.
Among other methods of abatement, one feasible and acceptable method to
correct this hazard is to:
1)Affix a legible label according to paragraph 16-2.1.1.4, ANSI
Standard B30.16, Overhead Hoist Standard.
c)Welding and Repair Shop, Union Drilling, Punxsutawney, PA -
Employee(s) using a CM lodestar Protector Hoist, serial #L-71807, and
shop made "A" frame support to lift steel pieces. Items such as the
following were not inspected for damage at intervals as defined in 16-
2.2.1.1 b.1 or as specifically indicated:
1)All controls and operating mechanisms for improper operation.
2)All safety devices for malfunction.
3)Deterioration or leakage in air systems.
4)Hooks for deformations, chemical damage, or cracks.
5)Load carrying ropes or chains.
Among other methods of abatement, one feasible and acceptable method to
correct this hazard is to:
1)Conduct frequent inspections as per paragraph 16-2.2.1.2, ANSI
Standard B30.16, Overhead Hoist Standard.
d)Welding and Repair Shop, Union Drilling, Punxsutawney, PA -
Employee(s) using a CM Lodestar Protector Hoist, serial #L-71807, and
shop made "A" frame support to lift steel pieces. Complete inspection of
the hoist was not performed at intervals as generally defined in 16-2.2.1.1
b.2 depending on it's activity, severity of service,, and environment, or
as specifically indicated:
1)Loose bolts or rivets.
2)Cracked or worn drums or sheaves.
3)Worn, corroded, cracked or distorted parts such as pins, bearings,
shafts, gears, rollers, locking and clamping devices.
4)Excessive wear on motor or load brake.
5)Excessive wear of chain, rope, load sprockets, drums, sheaves,
and chain stretch.
6)Hooks.
7)Electrical apparatus.
8)Hook retaining nuts or collars and pins.
9)Supporting structure.
10)Warning label for absence or illegibility.
Among other methods of abatement, one feasible and acceptable method to
correct this hazard is to:
1)Conduct periodic inspections as per paragraph 16-2.2.1.3, ANSI
Standard B30.16, Overhead Hoist Standard.
