Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to amputation hazards and crush injuries foot pedal and foot
treadle bar
mechanisms that were not guarded to prevent the unintentional operation,
activation,  and/or
cycling of machines and/or machine components:
A.) Shop Area/Cincinnati Shear:  The foot treadle bar that extended across
the lower
front side of the shear was utilized to operate the machine and was not
guarded.
B.) Shop Area/Chicago Press Brake:  The press brake's foot pedal was not
guarded.
Among other methods, feasible and acceptable methods to correct the
conditions
noted
above include: (A) Shear - Installing a shield above the entire length of
the foot treadle
bar (B) Press Brake - Equipping the foot pedal with a  treadle guard (i.e.
inverted U-
shaped guard).
