periodic and regular
inspections was not established and followed for the purpose of
determining that all components
of the Company's Powdered Metal presses were in a safe operating condition
and adjustment
throughout the facility::
a.) Press Molding Department - The Company had not developed a program of
periodic and
regular inspections for the Powdered Metal presses throughout their
facility.  These presses
included, but were not limited to, Press #15, Press #21, Press #7, Press
#4, Press #19, Press
#18, Press #22, Press #14, Press #6, Press #17, Press #23, Press #24,
Press #25, and Press #26.
The ANSI STD B11.16-2003 Paragraph 9.1.1.3 requires procedures to be
developed to ensure
safe operating conditions.
FEASIBLE ABATEMENT METHODS:
1.)Consider the supplier's written instructions and recommendations in
determining
maintenance and inspection programs.
2.) Establish a system of periodic and regular inspections on all parts,
auxiliary equipment,
and safeguards.
3.) Inspect the condition of the clutch/brake mechanism, anti-repeat
feature and single stroke
mechanism weekly.
4.) Maintain records of inspections and maintenance.
