Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation and crushing injuries because a program of periodic and regular inspections was not established and followed for the purpose of determining that all components of the Company's Powdered Metal presses were in a safe operating condition and adjustment throughout the facility:: a.) Press Molding Department - The Company had not developed a program of periodic and regular inspections for the Powdered Metal presses throughout their facility. These presses included, but were not limited to, Press #15, Press #21, Press #7, Press #4, Press #19, Press #18, Press #22, Press #14, Press #6, Press #17, Press #23, Press #24, Press #25, and Press #26. The ANSI STD B11.16-2003 Paragraph 9.1.1.3 requires procedures to be developed to ensure safe operating conditions. FEASIBLE ABATEMENT METHODS: 1.)Consider the supplier's written instructions and recommendations in determining maintenance and inspection programs. 2.) Establish a system of periodic and regular inspections on all parts, auxiliary equipment, and safeguards. 3.) Inspect the condition of the clutch/brake mechanism, anti-repeat feature and single stroke mechanism weekly. 4.) Maintain records of inspections and maintenance.
