Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and amputation injuries during the operation of powdered metal (P/M) presses provided with inadequate barrier guards, improperly adjusted interlocks, and/or readily removable fasteners: a) Production Area - Gasbarre P/M molding press #S1, full revolution, serial #88297, 40 ton, interlocked barrier guard was not installed. The interlock safety device was bypassed in the setup mode. Access at the front of the press was measured at twelve inches (12") wide x twelve inches (12") height and within eight inches (8") of the point of operation. b) Production Area - Gasbarre P/M molding press #S2, full revolution, serial #91450, 60 ton, interlocked barrier guard was not installed. The interlock safety device was bypassed in the setup mode. Access at the front of the press was measured at twelve inches (12") wide x twelve inches (12") height and within eight inches (8") of the point of operation. c) Production Area - Gasbarre P/M molding press #S3, full revolution, serial #96754, 45 ton, interlocked barrier guard was not installed. The interlock safety device was bypassed in the setup mode. Access at the front of the press was measured at twelve inches (12") wide x twelve inches (12") height and within eight inches (8") of the point of operation. d) Production Area - Gasbarre P/M molding press #M1, part revolution, serial #88296, 45 ton, had no barrier guard to protect an front opening on the adjustable barrier guard two inches (2") wide x six inches (6") long and ten inches (10") from the point of operation. e) Production Area - Gasbarre P/M molding press #M11, part revolution, serial #93519, 30 ton, had an front opening on the adjustable barrier guard two inches (2") wide x five inches (5") long and ten inches (10") from the point of operation. f) Production Area - Gasbarre P/M molding press #M16, part revolution, serial #94589, 200 ton, had an front opening on the adjustable barrier guard three inches (3") wide x eight inches (8") long and thirteen inches (13") from the point of operation. g) Production Area - Gasbarre P/M molding press #M18, part revolution, serial #95659, 60 ton, had an front opening on the adjustable barrier guard two inches (2") wide x six inches (6") long and eleven inches (11") from the point of operation. h) Production Area - Gasbarre P/M molding press #M21, part revolution, serial #9889, 200 ton, had a front opening on the adjustable barrier guard fifteen inches (15") wide x thirty one inches (31") long and thirteen inches (13") from the point of operation. FEASIBLE ABATEMENT METHODS: 1. Utilize a fixed barrier guard to prevent entry of hands into the point of operation by reaching through, over, under, or around the guard and remove readily removable fasteners in accordance with ANSI B11.16-2003, section 8.4, Point of Operation Guards. 2. Secure and mount the barrier guard to minimize the possibility of misuse or unauthorized removal in accordance with ANSI B11.16-2003, section 8.3, Guards.
