were exposed to working from a elevated work platform without a properly
installed emergency
escape line and Geronimo device:
a) Crawford County, Vernon Township, Scott Unit #4 well - On or about July
14, 2005
the emergency escape Geronimo device was not installed on the derrick
board. The
derrick board was located sixty-five feet (65') above the ground.
ABATEMENT NOTE:
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to provide an emergency escape means (such as a Geronimo Line)
for any
employee working from the derrick board, as specified by Section 6.10.1 of
the
American Petroleum Institute (API) Standard RP 54-1999, "Recommended
Practices
for Occupational Safety and Health for Oil and Gas Well Drilling and
Servicing
