Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to:  Hazards of falling from ladders. At the Toyota Center of
Houston Tx an employee
of a subcontractor was using a 32 foot metal ladder to access the Houston
Rocket's scoreboard
at a height of 25 feet. The ladder was not tied off/ Secure  at the top
and the ladder was placed
on an ice foundation (Hockey Rink). The employee attempted to decent from
the ladder and fell
to his death.
Among other means, one feasible and acceptable abatement to correct the
hazards is to
insure that the employee are properly trained on the use of ladders,
insure that the ladder
is secure/ tie off at the to prevent slippage on slippery surfaces(ice)
and follow ANSI
standards for metal ladders. Section 8.1.3 and table 22.
